Neurological Physical Therapy Clinic in Solana Beach

A dedicated space for stroke and brain injury rehabilitation — where focused, intensive therapy produces real results.

Introduction

Recovery from stroke or brain injury requires more than good intentions. It requires the right environment, the right tools, and a therapist who specializes in exactly what you're facing.

My clinic in Solana Beach is built around one purpose: helping people with neurological conditions recover as much function as possible.

I'm Spencer Goff, a Doctor of Physical Therapy and trained Neuro-IFRAH practitioner. Whether you've recently left inpatient rehab or you're a long-term patient looking to make more progress, my clinic offers the focused, hands-on care that neurological recovery demands.

Why a Dedicated Clinic Setting Makes a Difference

There are real clinical advantages to doing your rehabilitation in a correctly-equipped, distraction-free environment — especially for neurological conditions.

Dedicated equipment. My clinic has the treatment tools that support intensive neuro rehab — and space to work on gait, balance, and functional movement without the limitations of a home setting.

Full therapist focus.
In a clinic, every element of the session is optimized for your recovery. There are no environmental variables to work around — just focused, uninterrupted therapy time.

Structured intensity.
For many patients, coming to a clinic creates a mental shift — this is dedicated recovery time. That structure and routine can reinforce commitment and drive better outcomes.

Progress tracking over time.
Seeing the same space session after session makes it easier to observe and measure real changes in how you move, how far you walk, how well you balance. Progress becomes visible and motivating.

Peer environment.
Being around others who are also working hard toward recovery can be quietly motivating in ways that are hard to replicate at home.

What a Clinic Session Looks Like

Every patient's program is different, built around your specific neurological picture and functional goals. A clinic session with me typically includes:

  • Gait and walking retraining — using the space and equipment available to improve the quality, speed, and safety of how you walk

  • Balance and Control — progressive challenges in a controlled environment where it's safe to push your limits

  • Upper extremity rehabilitation — working to restore arm, hand, and fine motor function using hands-on Neuro-IFRAH approach

  • Trunk and core Control — building the proximal strength and control that underlies all movement

  • Functional task practice — simulating real activities so gains transfer to your daily life

  • Therapeutic activity progression — a structured program that evolves as you improve

Sessions are hands-on and active. I use the Neuro-IFRAH approach throughout — focused on retraining your nervous system to produce more normal, more natural movement, not just compensating around deficits.

Is Clinic-Based Therapy Right for You?

Clinic sessions tend to be the best fit for patients who:

  • Are medically stable and able to travel to appointments

  • Want access to therapeutic equipment and space

  • Prefer a structured, dedicated environment for their rehabilitation

  • Are working on higher-level goals — faster walking, stair climbing, return to activity

  • Live within reasonable distance of Solana Beach

  • Thrive with routine and the separation of "recovery time" from home life

Many patients also combine clinic and in-home sessions depending on what they're working on. I'm happy to discuss what structure makes the most sense for you.

Clinic or In-Home? Here's How to Think About It

Both settings have real advantages, and the right choice depends on where you are in recovery and what you're working toward.

Clinic Based Therapy

Full equipment and treatment space

Structured, distraction-free sessions

Higher-intensity progressive programs

Great for higher-level functional goals

In-Home Therapy

Therapy in your own environment

Family and caregiver can be involved

No transportation required

Ideal for early recovery or limited mobility

Not sure which is right for you? I offer both — and many patients do a combination. Reach out and we can talk through what would serve you best.

What Patients Say

"Spencer got my husband to stand, then walk with a walker, then take steps without one. A week ago, my husband played 18 holes of golf. I cannot give enough praise - he customizes activities to his clients' specific needs and is creative, enthusiastic, and patient. I don't know where we would be without him."

"Almost 3 years ago my husband suffered a massive stroke and is paralyzed on his left side. Spencer was recommended to us at that time to assist in my husband's recovery. To say I don't know what we would have done without him is a total understatement. Not only is he expertly skilled in physical therapy but he is a truly kind and compassionate person. He has gently guided my husband in his therapy through exercise and education of his new physical limitations. I on the other hand felt like I had been hit by a bus. I did not know anything about my husband's illness or the practical workings of hospitals, doctors and daily life of our new normal. Spencer accompanied us to doctor appointments and hospital visits. He interacted with the doctors in order to explain to me what was going on. Anyone who hires Spencer will be getting not only a highly skilled and knowledgeable physical therapist but also a very kind and caring person. I recommend him highly."

"I cannot recommend Spencer highly enough. After GBS-induced neuropathy left my feet severely impacted, I worked with Spencer for 10 months of in-home Neuro-IFRAH therapy. He is truly an expert in the field—knowledgeable, professional, and incredibly patient. What sets Spencer apart is his holistic approach; he was not only focused on my physical recovery but also kept me motivated with his kindness and humor. Thanks to Spencer’s tailored, in-home care, I have regained near 100% mobility and feeling in my feet. I no longer need PT, and am now back to doing all the activities I love. If you are facing neurological recovery, Spencer is the partner you want!"

"Spencer has been working with my husband for over 1 year and we can't say enough good things about him. He is very knowledgeable, intelligent, caring and supportive. He is really making a huge difference in my husband's life after stroke."

"Spencer has been an essential part of my recovery. He is patient with me and knowledgeable about his work. I have made marked improvements with Spencer and highly recommend his services."
